Your system of quality management under ISQM 1 and ISQM 2, step by step
This manual shows how an audit firm runs its system of quality management in SQMTrove: setting up the firm and its people, assessing quality risks and designing responses across the components of ISQM 1, running the annual independence campaign, keeping the client and engagement registers, performing engagement quality reviews under ISQM 2, monitoring, finding root causes and remediating, dealing with inspections by the regulators and with complaints, and signing the annual evaluation — then producing the inspection-ready pack.
It follows one fictional firm, Okafor Bello & Co, Chartered Accountants, from the day it opens SQMTrove in January 2026 to September 2026: the risk assessment, the busy season, an inspection by the Financial Reporting Council of Nigeria (FRC), a practice monitoring visit by the Institute of Chartered Accountants of Nigeria (ICAN), remediation and the annual evaluation as at 30 June 2026. The firm audits a listed bank, an insurer, a pension fund administrator, a listed manufacturer, large private companies, a foundation, a federal agency and small companies, so the manual shows how the rules differ by category of client. Every screenshot was taken from SQMTrove running that illustration, so the figures in the text match the screens.

A partnership of chartered accountants founded in 1994, registered with the FRC (firm registration FRC/2014/ICAN/00000001234, illustrative) and ICAN (ICAN/F/00417, illustrative). The clients, by category. What SQMTrove requires of an engagement follows from the client's category on the client record: whether it is a public interest entity (PIE) or publicly traded, whether a review is required by law or firm policy, and the risk rating.

The regulators. The firm is monitored by a government regulator and a professional regulator. The FRC registers audit firms and inspects PIE audits and firms' systems: it visits from 9 to 20 February 2026, reviews the 2024 audits of the bank and the insurer, and reports on 30 April with three findings. ICAN licenses practices and reviews them through its practice monitoring programme: it visits from 13 to 16 April and reports on 12 June with two recommendations. What the FRC and ICAN do in the illustration is fictional.

11. Monitoring: plan, activities and inspections
Approve the monitoring plan, schedule inspections and tests of responses, follow the engagement partner inspection cycle, and inspect engagement files with SQMTrove's checklist.

Prepare the evaluation as at the evaluation date from frozen evidence, conclude on each component and overall under ISQM 1.54, and sign it as the individual with ultimate responsibility.
